Use this online form ONLY if your membership list is stored electronically (such as in Excel or Word). You will be asked to attach that file at the end of this form. If your membership list is not electronic, use Form C under "Membership Forms - Printable."
This includes active life members and new members.
The number of paying members is the number of total club members minus the number of life members.
The amount due is equal to (the number of paying members * $10) plus the (number of new members *1)
Max. file size: 256 MB.
Please attach a file with a spreadsheet or table that lists all current members.
In the first column, use the following codes to indicate any:
C - changes in address, name, etc.
D - members who have been dropped
L - life members
M - members who are deceased
N - new members

The other columns should be: Last Name, First Name, Address, City, State, ZIP, *Phone, *Email, Other Club. *new this year

Under "Other Club," list any other clubs to which the member belongs.

At the end of your list, include the names of any life members who have become inactive.
