In the first column, use the following codes to indicate any:
C - changes in address, name, etc.
D - members who have been dropped
L - life members
M - members who are deceased
N - new members
The other columns should be: Last Name, First Name, Address, City, State, ZIP, *Phone, *Email, Other Club. *new this year
Under "Other Club," list any other clubs to which the member belongs.
At the end of your list, include the names of any life members who have become inactive.
